The Care Coordination Assistant is responsible for selected functions related to Advance Directives, Utilization Review Education and Disposition Planning. The Care Coordination Assistant communicates and collaborates with Care Coordinators and other disciplinary team members. The Care Coordination Assistant’s responsibility is to support the Care Coordinators and patients requesting assistance with advance directives. Support duties may include assisting with utilization management, transition of care, pharmaceutical applications and other duties as assigned by Supervisor. The Care Coordination Assistant may also serve as an educator to patients regarding disease processes and essential follow up care related to their diagnosis.
Please note that this is a PRN position.
Required Minimum Education
Bachelor’s degree in related field (Bachelor’s in Social Work preferred). Or LPN license in Georgia.
Required Minimum Experience
A minimum of one-year experience in a healthcare environment preferred
Required Minimum Skills
Professional organizational, verbal and written communication skills a must. Ability to interact appropriately with patients from various backgrounds.
