Responsibilities

The home hospice Supervisor is a licensed professional nurse who is a member of an interdisciplinary team serving the terminally ill patients in the hospice home care program. Provides supervision of the Hospice RN case managers and the Certified nursing assistants on the team. Assesses the competencies and skills of the team members, supports their orientation and training during onboarding and thereafter. Performs co-visits to evaluate performance . Mentors new employees, ensures their compliance with policies and procedures and adherence to the Plan of Care developed by the team. Makes assignments , provides coverage for absences. The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence based practice and research.

Qualifications

Required Minimum Education: Graduate of an accredited school of nursing. BSN preferred. Required Minimum Licensure/Certification Current Registered Nurse license in the state of Georgia.

Required Minimum Experience: At least two years of acute care experience, at least one year home hospice experience, leadership experience preferred. CHPN certification preferred.

Required Minimum Skills: Emotional maturity, strong interpersonal skills, effective written and oral communication skills, Excellent clinical assessment skills, Possesses knowledge of developmental characteristics of people of all ages and the holistic approach to care, Possesses knowledge of a wide range of disease processes , competent in documentation in EMR.
