The Discharge Call Nurse makes telephone contact with patients within 24-48 hours after discharge to assess patient
satisfaction, evaluate patient education, determine patients' compliance with discharge instructions and provide
follow up support/instructions as needed. This position identifies trends that may require improvement of practice,
quality of patient care or hospital process improvement. The DCN represents Administration to WellStar patients,
families, and visitors in communicating relevant policies, responding to conflict situations, and providing service
recovery. The nurse responds to difficult customer service situations and supports families and staff. At all times, the
DCN models exemplary customer service and professionalism.
Required Minimum Education: Graduate of an accredited/approved school of nursing:
Baccalaureate degree in nursing (BSN) from an accredited school of nursing preferred;
Current/active license as a registered nurse in the State of Georgia
Required Minimum Experience: Minimum 5 years current experience direct patient care in Inpatient acute-care,
call center experience preferred.
Required Minimum Skills: Proficient in MS Word, PowerPoint, and Excel. Exemplifies superior level of customer
service. For Direct Patient Care positions, Indicate American Heart Association or American Academy of
Pediatrics certification required: BLS (Basic Life Support), ACLS ( Advanced Cardiac Life Support), NRP Neonatal Resuscitation Program, PALS Pediatric Advanced Life Support
