Responsibilities

WellStar is looking to add a Team Lead Outpatient Case Manager to its Population Management team!!

The Team Leader (TL) is responsible for coordination and function of a team across the continuum during the

patient’s acute, chronic and long-term stages of illness for a defined patient population.

The TL position will lead an effective Ambulatory Care Management program that includes Disease

Management, Wellness, and Prevention and Triage. Responsibilities include operational planning consistent

with existing policies and/or principles; managing medical management activities to include tracking, trending

and analyzing clinical outcomes and utilization data; and working in collaboration with population health leaders,

medical director, ambulatory care managers and hospital case management departments. The TL will

participate in the development of the quarterly payer reports and program goals, implementation of corrective

action plans related to deficiencies and requirements. The TL role also includes collaborating with the Quality,

Compliance and OL Department to effectively integrate and implement processes consistent with requirements.

The TL develops and implements new programs under the direction of the manager and Population health

team.

The TL of Case Management is part of the Integrated Care Management, with focus is on safe and sustainable

care transitions that are efficacious, and which lead to quality outcomes: ensuring patients are in the right care

environment, at the right time, and for the right costs. This includes utilization management, transitional care

planning, psychosocial and functional status assessment, patient advocacy, education for the patient’s /family,

and monitoring quality indicators to demonstrate outcomes for the team resulting from the service provided.

Collaborates with all team members to provide a comprehensive assessment the patient’s plan of care,

goal/outcome attainment and continued care needs.

Qualifications

Required Minimum Education: RN with a Georgia License, Graduate of an accredited/approved school of nursing: Baccalaureate degree in nursing (BSN) from an accredited school of nursing preferred, or MSW from an Accredited School of Social Work

R equired Minimum Licensure: RN with a Georgia License or LMSW with Georgia License

Required Minimum Experience: Three (3) to five (5) years of Clinical Practice/Experience is required, with one to two years of Case Management/Care Coordination experienced preferred.

Required Minimum Skills:

• Knowledge of Population Health and Case Management processes.

• Excellent organizational and professional communication skills.

• Knowledgeable in utilizing screening criteria in review of clinical and claims data.

• Strong ability to engage physicians and clinical staff to effectively achieve desired goals.

• Ability to manage fast pace changes. Knowledge of wide variety of local and national resources for use in Care Management process.

• Strong organizational skills with the ability to work well under pressure with conflicting priorities.

• Effective interpersonal skills: strong verbal, written and presentation skills

• Proficiency with Microsoft office suite products.
